Responsibilities for Home Health / Home Care Visiting Nurses – Adult include but are not limited to:

• Provide direct patient care to patients residing in the Staten Island, Brooklyn, Queens, Bronx and Manhattan boroughs.
• Coordinator of care, follow-up on changes regarding patient status, medications and assessing services the patient requires.
• Weekend and Holiday nurses will administer care according to MD orders and report changes to MD.
• See, plan and document their care / visits including the weekend. 
• Responsible for submitting billing information accurately and timely.
    

 Requirements for Home Health / Home Care Visiting Nurses – Adult include but are not limited to:

• Current and unrestricted RN nursing license in NY
• BSN  Degree preferred
• Must have a minimum of two years home care / home health experience 
• Must be experienced in OASIS documentation
• Must be eligible to work in the United States
• Availability: Should be available at least three days a week and/or have weekend availability
